چکیده
The patient had a clinical history of stable coronary artery disease, hypertension, hypercholesterolemia, and chronic (non-dialytic) renal insufficiency with baseline creatinine clearance of 20 mL/min. His surgical history included cardiac revascularization at the age of 45 years, and left internal carotid angioplasty and stenting after a stroke episode in 2004, which left no persistent deficits. Recent myocardial perfusion scintigraphy showed no signs of myocardial ischemia except for fibrosis in the inferior wall; gated SPECT revealed mild left ventricular dysfunction (EF = 48%). In addition, the patient had an asymptomatic abdominal aortic transrenal aneurism (7 cm), for which surgical intervention had been postponed following the recent neurological events.

ANGIOPLASTY AND STENTING OF CAROTID ARTERY STENOSIS WITH EMBOLIC PROTECTION DEVICES
Background: Carotid artery stenting (CAS) has recently been recommended as an alternative to carotid endarterectomy (CEA) by some clinicians. Objective: This study was designed to evaluate the success rate and in-hospital and 30-day adverse events in our first experiences in Iran for CAS with protection devices, to document our results and guide further use of CAS. Methods: From December 2...
متن کاملCarotid angioplasty and stenting in a patient with high grade stenosis of Internal Carotid Artery associated with both vertebral arteries and contralateral carotid occlusion
Severe internal carotid artery (ICA) stenosis is a common cause of cerebrovascular accident (CVA) in middle-aged patients. Contralateral carotid occlusion (CCO) in patients with severe ICA stenosis is associated with high risk of CVA. Carotid endarterectomy (CEA) is associated with more complications in patients with CCO than those without CCO. In this study, we present the case of a 61-year-ol...
متن کاملAssociation between high sensitivity C-reactive protein (hs-CRP) levels and the risk of major adverse cardiovascular events (MACE) and/or microembolic signals after carotid angioplasty and stenting
Background: To evaluate the association between pre/post-procedural high sensitive C-reactive protein (hs-CRP) level and hs-CRP difference, and the risk of major adverse cardiovascular events (MACE) or new diffusion-weighted MRI lesions after carotid angioplasty and stenting (CAS). Methods: In this study, conducted in 2016 in Shiraz (Iran), patients who underwent diagnostic angiography and CAS...
متن کاملPatient State Index Detects Changes in Cerebral Blood Flow and May Predict New Ischemic Lesions during Carotid Stenting—Case Report
Carotid angioplasty and stenting (CAS) was developed to be a less invasive and complex procedure compared to carotid endarterectomy (CEA). It has emerged as an alternative for patients who are considered to have high surgical risks due to medical comorbidities or anatomical high-risk features [1]. The procedure is usually done under local anesthesia with light sedation, with the subsequent expe...
متن کاملNEUROSCIENCES AND NEUROANAESTHESIA Carotid artery disease: stenting vs endarterectomy
Key points † Carotid endarterectomy is a standard intervention for carotid artery stenosis, while angioplasty/stenting is gaining acceptance. † General and regional anaesthetic techniques are equally effective for CEA; CAS is performed under sedation. † It is important to optimize cerebral blood flow and minimize myocardial stress during these procedures. Summary. Several multicentre, randomize...
